While heat action plans have proliferated across states and cities, their framework remains fundamentally disconnected from core public health principles that could save thousands of lives each year.

The Growing Heat Crisis

India has witnessed a dramatic increase in extreme heat events over the past two decades. Temperatures regularly soar beyond 45 degrees Celsius in multiple states during summer months, with urban heat islands pushing the mercury even higher in densely populated cities. Official statistics often underreport heat-related deaths, but estimates suggest that thousands of Indians die annually from heat exposure, with countless more suffering from heat-related illnesses.

The most vulnerable populations include outdoor workers, elderly citizens, children, pregnant women, and those with chronic health conditions. What Current Heat Action Plans Get Wrong

Most Indian heat action plans operate on a warning and advisory model. They issue color-coded alerts, recommend people stay indoors during peak hours, and suggest drinking plenty of water. While these measures have some value, they fail to address the fundamental public health challenge: millions of Indians cannot simply avoid heat exposure.

The approach treats heat as a weather event requiring behavioral adjustments rather than a health emergency demanding systematic medical and social interventions. This misses several critical public health dimensions.

First, current plans rarely integrate with primary healthcare systems. There is limited training for healthcare workers to recognize and treat heat-related illnesses at early stages. Heatstroke symptoms often mimic other conditions, and delayed diagnosis can prove fatal. Second, surveillance systems remain inadequate. Robust public health responses require real-time data on heat-related hospital admissions, emergency room visits, and mortality patterns. Most Indian states lack this granular, timely information, making it impossible to track effectiveness of interventions or identify emerging hotspots.

The Missing Public Health Infrastructure

A genuine public health approach would establish cooling centers in every neighborhood, particularly in low-income areas where home cooling is unavailable. These facilities would need to be accessible, well-publicized, and adequately staffed with basic medical support.

Occupational health regulations represent another glaring gap. While some states have introduced guidelines for work breaks during extreme heat, enforcement remains virtually non-existent. Construction sites and factories rarely provide shaded rest areas, adequate drinking water, or medical monitoring for workers showing heat stress symptoms.

Urban planning also falls outside the purview of most heat action plans, despite its crucial role. The absence of tree cover, prevalence of heat-absorbing surfaces, and lack of green spaces significantly amplify heat exposure in cities. Public health logic demands integration of cooling considerations into infrastructure development.

What Evidence-Based Heat Response Looks Like

Effective heat-health action plans, as demonstrated in countries like Australia and France, operate on multiple levels. They establish clear clinical protocols for diagnosing and treating heat illness across all healthcare facilities. They create mandatory cooling access requirements for vulnerable populations. They enforce workplace protections with penalties for violations.

These systems also invest in predictive analytics, using weather forecasts combined with demographic data to identify highest-risk populations and pre-position medical resources. Community outreach workers proactively check on elderly and isolated individuals during heat events.

The Way Forward

India needs to fundamentally reimagine heat response as a public health emergency rather than a seasonal inconvenience. This requires several concrete steps.

State health departments must develop standardized clinical protocols for heat illness and train all healthcare workers in their application. Surveillance systems need urgent upgrading to capture heat-related morbidity and mortality in real-time. Every district should map its vulnerable populations and establish targeted intervention programs.

Regulatory frameworks must mandate cooling access in workplaces and public spaces, with meaningful enforcement mechanisms. Urban development policies should incorporate heat mitigation through green infrastructure and reflective building materials.

The current advisory-based approach costs thousands of lives annually while creating a false sense of preparedness. Until India's heat action plans embrace comprehensive public health logic, they will remain inadequate to the challenge of protecting citizens from an increasingly dangerous climate reality.
